EMPOWER Structured diabetes education

WORKING WITH THE NHS TO INCREASE ACCESS TO STRUCTURED DIABETES EDUCATION

Only 7.6% of people newly diagnosed with type 2 diabetes are recorded as attending SDE1… But Spirit Healthcare has a solution!

We are now delivering our EMPOWER Structured Diabetes Education programme across the country.

EMPOWER courses currently have an uptake rate of 44%2 and 99% Friends and Family Test score3.

With EMPOWER we keep it really simple - you don’t need to worry about finding any additional staff.

We employ a dedicated admin and clinical educator team to:

  • Process fax, electronic and self-referrals – with a dedicated 24/7 support line for patients.
  • Provide courses across a range of local venues – during the week, in the evening and at weekends, seeing up to 12 patients per session.
  • Contact every patient to help them book onto a course that is convenient for them.
  • Collect quality of life and clinical outcome data before and after the course.

The EMPOWER programme is led by our Clinical Lead, educated to Masters level and with over 30 years experience in diabetes nursing.

All educators are trained to deliver EMPOWER and are subject to annual reassessment.

The EMPOWER programme was developed using an evidence based approach according to NICE criteria, is underpinned by a written structured curriculum.

EMPOWER is QISMET certified. QISMET’s Diabetes Self Management Education (DSME) Quality Standard is a way for providers to demonstrate they meet relevant national criteria, including the NICE Guidelines (e.g. NG28).

EMPOWER is subject to regular External Quality Assurance.
